Getting to Know and Manage Your Patients
Primary care practices typically collect a lot of information from their patients. We ask patients about their health and social histories. This expanded to tobacco, alcohol and potential drug use as contributors to health status as well as income and employment for purposes of health insurance eligibility. Now many practices ask questions about other social care needs that correlate to the social determinants of health. The routine demographic data, comprehensive health assessment, medication reconciliation and others provide you with a more complete picture. This helps your care team as you obtain a more comprehensive and realistic overview of how the external world impacts a patient, and how the patient lives in that world. Your use of evidence-based care clinical decision support tools helps you better support your patients and the population that you serve.
- Discuss how, when and who connects with the patient to get to know more about their past, present and future health care needs from a wholistic perspective.
- Explore how your patient diversity data can help you meet the needs of your most vulnerable patients.
- Identify strategies used to address patient needs via proactive outreach and how your use of internal and external resources supports this activity.
