Primary Care - Reaching for Excellence via the Medical Home & Related Initiatives
Kentucky primary care practices are encouraged to demonstrate improvement with clinical quality metrics, access to care indicators, HIT use, response to the pandemic and provision of related services as well as other measures. You work to identify, monitor, and follow-up with patients who have diabetes or another chronic condition. You work to improve the delivery of recommended preventive services for all of your patients. Health Resources Service Administration (HRSA) Bureau of Primary Health Care (BPHC's) Uniform Data System (UDS) Community Badges and the Reaching for Excellence Advancing Community Health (BPHC REACH) establishes an operating model for health centers to adopt. The PCMH model of care is one framework to utilize as a means to demonstrate your commitment to continuous quality improvement.
