Social Determinants of Health’s (SDOH’s) Impact on the Patient Centered Model of Care
The patient-centered model of care encourages the use of patient level social care needs information as a means to provide better care and improve health outcomes for populations. Making social determinants of health (SDOH) a strategic priority at your health center takes time and is an investment. In doing so, a primary care practice needs to know about the available community resources, and gaps, that can help support the needs of patients. SDOH best practices and initiatives can be considered to help you develop your own strategy.
Session Objectives:
Discuss how a practice can start to assess, support, and/or expand how SDOHs in the population they serve are addressed.
Understand the NCQA PCMH criteria requirements and recommendations related to SDOH.
Identify community resources your practice can promote and collaborate with to support your patient population.
