Full Job Description


The Healthcare for the Homeless Nurse Specialist will be part of an interdisciplinary team care team in a highly collaborative environment within a patient centered, integrated care healthcare organization. This position will be working within the Healthcare for the Homeless program and will be responsible for facilitating care coordination of patients across Health Care for the Homeless programs including Primary Care, Medical Respite and Outreach programs. Patients are served in clinics, the community, and in partner organizations or shelters. This position will als assist the Director of Homeless Services in the administrative and clinical oversight for HealthFirst Bluegrass Inc. Health Care for Homeless services and program.

  • Works collaboratively with care team to coordinate care including case management, triage, walk-in services, medication refills, phone calls and support services for medical provider in clinic and participating in team huddles.
  • Perform HCH RN coordinator duties, including direct patient care in a variety of healthcare settings.
  • May assist HCH Director in assuring that HCH staff are proficient in all required clinical skills
  • Identifies and refers patients having problems related to the health care system, financial or social barriers (e.g., transportation services or prescription assists) and/or needing to be linked with community resources to Community Health Worker
  • Assist with backup VFC/Vaccine storage and handling
  • Assists with data collection and generation of patient registry reports as needed
  • Ensures clinic remains in compliance with local, state, and federal regulation
  • Adhere to attendance and Leave Policies and Procedures, and demonstrate flexibility in workhours to meet operational requirements


  • Registered Nurse with three years of experience as a clinical nurse or ·
  • Master’s Degree in Nursing or other related field plus one year of nursing experience.

Job Type: Full-time

Pay: $30.00 per hour
